About Our Facility

Comprehensive assessment of needs of the frail older adult, referral to services needed, assistance with service linkage, and case management. Also provided is a Protective Service Program for abused and neglected seniors, the Nursing Ombudsman Program to advocate for residents and resolve their problems, and The Daily Money Management Program to assist seniors in their bill paying activities.

Age requirements:

Age 60 & over. Program is designed to serve the frail older adult with multiple problems.
