AM I ELIGIBLE FOR HOME HEALTH? To receive services under Medicare/Medicaid, the following guidelines must be met:
. Patient must require the services of an RN or therapist.
. Patient leaves his/her home infrequently or with great effort.
. Physician must order home care services.
COORDINATED SERVICES - Sherrick Home Health is supported by Sherrick Drug & Medical's pharmacy, home infusion, and durable medical equipment divisions. Our services are situated in one building which is not only convenient for our customers, it also helps our staff work together to provide comprehensive, coordinated home care. One single call can take care of all home care arrangements.
DISCHARGE ASSISTANCE - The home health nurses from Sherrick Home Health work directly with physicians and hospital staff members to ensure smooth transitions from hospital care to home care. Whenever possible, a Sherrick's nurse meets with the patient for a pre-hospital discharge informational meeting about home care.
PATIENT ASSESSMENT - A trained staff member meets with and assesses the needs of each patient with respect to the services and treatment provided. An individualized plan of care is then developed by the patient's primary nurse and personal physician which is designed to meet the unique needs of the patient. Continually monitored, the plan of care is updated frequently to ensure care that is appropriate.

REIMBURSEMENT MANAGEMENT - Free insurance verification and filing is available for all Medicare, Medicaid, and private insurance clients. In addition to traditional payment methods, Sherricks is also willing to explore alternative payment solutions, so more patients may receive the benefits of home care.
